Lack of a good control system
Not all control systems are created equal and no one control system can solve all problems. Installing a well thought out control system does not mean it will perform well.
A typical missing component of underperforming control systems is the quality of the installation. Testing, Verification and Commissioning of a control system is extremely important and the only way to achieve your goals. It also provides a baseline of the control system performance for future comparison.
